Below are links to invitation forms for both dependents and for visitors.
F-2 & J-2
This form is to be used by those students and scholars who would like to invite a dependent (spouse or minor child/children) to join them in the U.S.
The Center for International Studies will issue the proper immigration document (I-20 or DS-2019) once the completed form has been received.
B2 Visitor Visa
This form is to be used by those students and scholars who would like to invite a family member for visitation purposes only.
The Center for International Studies will issue an invitation letter to be included with the individual’s B2 visitor visa application.
